The episode opens with (played by Tetsuo Kurata) on the run through a dark, industrial landscape, pursued by the three sinister High Priests of the cult Gorgom : Darom, Baraom, and Bishium.

The second half of the episode is a desperate flight. Kotaro, now possessing the Kingstone in his belt, transforms for the first time. He doesn't look heroic. He staggers, confused, wearing a suit that looks like obsidian insect armor. He fights a mutated Gorgom monster called the "Bat Mutant" not with joy, but with primal survival instinct. The episode ends not with a victory pose, but with Kotaro wandering Tokyo, realizing he is no longer human, whispering the line that would define the series: "I am a cursed warrior... Kamen Rider Black."
