Some homebrew applications and trainers (cheat codes) are designed specifically to work with the XEX folder structure rather than the raw ISO format.
The ISO is corrupted, or it is an Xbox One/Series X|S backup. Fix: Re-download the ISO or verify it using abgx360 software.

On a modded console, you don't need a shortcut—just run default.xex via File Manager (XeXMenu or Aurora). But for emulators like Xenia, simply double-click default.xex in Windows (if associated) or drag the folder into Xenia.
